布地奈德雾化吸入对肺炎支原体肺炎患儿临床症状、肺功能及体液免疫功能的影响

摘要:
目的:探讨布地奈德雾化吸入对肺炎支原体肺炎(MPP)患儿临床症状、肺功能及体液免疫功能的影响。方法:选取2014年9月至2015年1月于我院住院的MPP患儿60例,随机分为对照组和观察组各30例。对照组给予阿奇霉素常规治疗(10 mg/kg,1次/天,用3 d停4 d),观察组在对照组治疗基础上加用布地奈德混悬液雾化吸入(每次2 mL,每天2次,间隔4~6 h)。测定患儿治疗前和治疗2周后的血清免疫球蛋白(IgG、IgA、IgM)水平(散射比浊法)、肺功能(肺功能仪),观察患儿临床症状及肺部体征改善情况。结果:观察组患儿治疗前和治疗2周后的血清免疫球蛋白(IgG、IgA、IgM)水平差异无统计学意义(P>0.05);咳嗽消失时间、干湿啰音消失时间短于对照组(P<0.05);治疗2周后肺功能指标FVC、FEF75改善率高于对照组(P<0.05)。结论:布地奈德雾化吸入对MPP患儿的体液免疫功能无影响,是MPP的有效辅助治疗方法之一,值得临床进一步观察研究。

Abstract:
Objective: To explore the effect of budesonide aerosol inhalation on clinical symptoms of mycoplasma pneumonia, pulmonary function and humoral immune. Methods: Collected 60 cases of children with mycoplasma pneumoniae pneumonia from September 2014 to January 2015. They are randomly divided into control group 30 cases and the experimental group 30 cases. The control group are given conventional treatment. The experimental group are given both the routine treatment and local atomization inhalation of budesonide (2 mL/time, 2 times a day). Using scattering turbidimetric method is used to measure the blood immunoglobulin (IgG, IgA, IgM) between the treatment of 60 cases before and after treatment for 2 weeks. And at the same time we test the lung function of the 60 cases in lung function room before and 2 weeks of treatment by the spirometer made in Germany. In the treatment of 60 cases of children with mycoplasma pneumonia, in the whole process, we closely observe of clinical symptoms and signs. The two deputy chief physician evaluate lung signs. Accurately recording the raw data and the statistical software SPSS16.0 were used to analyze the test data. Results: (1) Duration of cough and lung signs turnaround time in the experimental group (14.80±1.47, 6.63±1.06) are significantly shorter than that in the control group (13.87±1.25, 5.77±0.68), the difference was statistically significant (P<0.05). (2) The immune globulin (IgA, IgG, IgM) in the experimental group is not obvious difference with that after two weeks, the difference was not statistically significant (P>0.05). (3) After two weeks treatment lung function of the experimental group such as FVC period (28.77±15.94), FEF75 period (56.95±62.74) is significantly higher than the control group (21.5±11.52, 26.80±16.07), the difference was statistically significant (P>0.05). Conclusion: Atomization inhalation hormone is a kind of auxiliary treatment method of mycoplasma pneumonia and it has an effect on humoral immunity of children.It needs to be further studied.
