| 目的:为保障新生儿肠外营养(PN)支持的安全性、有效性提供参考。方法:统计我院静脉药物调配中心(PIVAS)2013年7月至2014年6月的PN处方(共计6 557张),从病种、PN处方常用药品品种、处方配比等方面进行分析,探讨新生儿PN合理性及稳定性的影响因素。结果: 6 557张PN处方中,病种符合新生儿PN适应证,药品品种选择合理;处方配比方面存在问题,其中热氮比不合理2 149张(32.77%),糖脂比不合理1 984张(30.26%),K+浓度不合理128张(1.95%),氨基酸与葡萄糖比例不合理922张(14.06%)。结论:我院新生儿PN处方使用情况基本合理,但处方配比方面存在问题。药师应严格审核处方,以保障临床新生儿PN的安全性和有效性。 |

| The Prescription Rationality Analysis of the Neonatal Parenteral Nutrition in Our Hospital |

| Objective: To provide the reference to ensure the safety and the effectiveness of the neonatal parenteral nutrition (PN) support. Methods: A total of 6,557 parenteral nutrition prescriptions in the pharmacy intravenous admixture services were selected from July 2013 to June 2014. The influence factors on the rationality and stability, PN commonly composition, ratio of prescription and so on were summarized and analyzed, to explore rationality and the factors affecting the stability of the newborn PN. Results: In the 6,557 parenteral nutrition prescriptions, diseases in neonatal parenteral nutrition conformed to indications, drug species selection was reasonable. Two thousands one hundred and forty nine cases were thermal nitrogen ratio unreasonable, accounted for 32.77%. One thousand nine hundreds and eighty four cases were glycolipid ratio unreasonable, accounted for 30.26%. One hundred and twenty eight cases were potassium ion concentration unreasonable, accounted for 1.95%. Nine hundreds and twenty two cases were irrational proportion of amino acid and glucose, accounted for 14.06%. Conclusion: PN prescriptions in our hospital were basically reasonable, but there were still some problems in the proportion of prescriptions. Pharmacists should strengthen check the prescription strictly to ensure the safety and effectiveness of clinical neonatal PN. |
