[摘要] 目的:探讨血清 C 反应蛋白(CRP)联合降钙素原(PCT)检测儿童感染性发热病因。 方法:选取132 例发热患儿,其中 74 例细
菌感染性发热患儿,58 例病毒感染性发热患儿。 测定患儿血清 CRP、PCT、白细胞(WBC)计数和中性粒细胞比例(N% )。 结果:
细菌感染性发热患儿 CRP、PCT、WBC 水平明显高于病毒感染性发热患儿,差异有统计学意义(P<0.05)。 两组患儿中性粒细胞
比例比较差异无统计学意义(P>0.05)。 CRP、PCT、WBC 诊断细菌感染性发热受试者工作特征曲线下面积(ROC-AUC)分别为
0.864、0.810、0.654。 CRP对儿童细菌与病毒感染性发热的辨别力最佳。 CRP( >11 mg/ L) 诊断细菌感染性发热的灵敏度为
86.5% ,特异度为69.0% 。 PCT(>0.37 ng/ mL)诊断细菌感染性发热的灵敏度为70.3%,特异度为89.7%。 平行诊断试验灵敏
度为96.0% ,特异度为61.9% 。 系列诊断试验灵敏度为60.8%,特异度为96.8%。结论:血清 CRP 和 PCT 能辅助鉴别儿童感
染性发热;二者联合检测可显著提高细菌感染引起发热的诊断的灵敏度(平行诊断试验)和特异度(系列诊断试验)。 |

[Abstract] Objective: To study serum C-reactive protein (CRP) and procalcitonin (PCT) for etiology diagnosis of infectious fever in
children. Methods: One hundred and thirty-two children with fever were chosed, and serum CRP and PCT were detected in 74 children
with infectious fever of bacteria and 58 children with infectious fever of virus. The levels of CRP and PCT, white blood cell count, the
proportion of neutrophils were detected. Results: The levels of CRP, PCT, WBC of children with infectious fever of bacteria were higher
than those in children with infectious fever of virus (P<0.05). The ROC-AUC of CRP, PCT and WBC diagnose infectious fever of bacteria
were 0.864, 0.810, 0.654.CRP showed the best discriminative power to differentiate between infectious fever in children.The sensitivity of CRP (>11 mg/L) and PCT (>0.37 ng/mL) for diagnosis of bacteria infectious fever were 86.5% and 70.3% and the specificity were
69.0% and 89.7% , respectively. In parallel diagnostic test, the sensitivity and specificity were 96.0% and 61.9%; in serial test, they were 60.8% and 96.8% , respectively. Conclusion: Serum CRP and PCT can help differentiate infectious fever in children. The
combination of CRP and PCT significantly improve the sensitivity of parallel diagnostic test and the specificity of serial test for diagnosis of bacteria infectious fever. |
