| 目的:分析我院临床药师参与临床会诊的特点及效果,为儿科临床药师选择开展药学实践的模式和方向提供参考。方法:采用回顾性研究方法,查阅2013-2016年我院临床药师参与的508例临床会诊记录,从申请会诊的科室、会诊原因、会诊意见及采纳情况、患儿预后等方面进行统计分析。结果:申请会诊的科室主要为新生儿医疗中心(322例,63.39%),会诊原因主要为预防真菌感染(324例,63.78%),会诊意见主要为制定个体化药物治疗方案(225例,44.29%)。会诊意见被临床全部采纳386例(75.98%),部分被采纳92例(18.11%),未被采纳30例(5.91%)。采纳会诊意见的病例治疗有效率88.91%(425/478),未采纳会诊意见的病例治疗有效率26.67%(8/30)。结论:儿科临床药师通过参与临床会诊,在优化药物治疗方案、保障儿童用药安全方面发挥着积极作用。 |

| Objective: To analyze the features and effects of clinical consultation participated by pediatric clinical pharmacists in our hospital, so as to provide reference for selection of clinical pharmacists in mode and direction of developing pharmaceutical practice. Methods: By retrospective analysis method, a total of 508 clinical consultations participated by pediatric clinical pharmacists in our hospital from 2013 to 2016 were analyzed statistically in respect of consultation departments, consultation reasons, consultation suggestions and adoption and prognosis of children, etc. Results: Consultation application was mainly from neonatal medical center (322 cases, 63.39%), the main reasons of consultation were to prevent fungal infection (324 cases, 63.78%), the main suggestions of consultation were to formulate individualized drug treatment plan (225 cases, 44.29%). Consultation suggestions were totally adopted in 386 cases (75.98%), partially in 92 cases (18.11%) but not adopted in 30 cases(5.91%). The effective rate of patients adopting consultation suggestions was 88.91% (425/478), and the data in patients without adopting consultation suggestions was 26.67% (8/30). Conclusion: By participating in clinical consultation, pediatric clinical pharmacists play an active role in optimizing drug treatment plan and ensuring the safety of medication in children. |
