儿童慢性鼻窦炎的细菌学分布与血清学指标的相关性分析

摘要:
目的:探讨儿童慢性鼻窦炎(CRS)的细菌学分布与血清学指标的相关性。方法:选择2015年8月至2018年2月在我院门诊及住院的CRS患儿78例(CRS组)及健康儿童(对照组)80例,采集两组儿童的鼻腔分泌物进行病原菌分离培养,检测两组患儿的血清IgE表达水平并进行相关性分析。结果:CRS组患儿鼻窦分泌物培养的细菌阳性率为44.9%,对照组为4.6%,CRS组显著高于对照组(P<0.05)。CRS组患儿中共分离出细菌50株,其中表皮葡萄球菌16株,产气肠杆菌10株,巴氏葡萄球菌6株,腐生葡萄球菌5株,浅绿气球菌5株,其他菌株8株。CRS组患儿的血清IgE阳性率为71.8%,对照组为1.3%,CRS组显著高于对照组(P<0.05)。Spearman相关分析显示CRS组患儿鼻窦分泌物细菌培养阳性与血清IgE阳性呈正相关(P<0.05),在对照组中无显著相关性(P>0.05)。结论:儿童CRS的鼻窦分泌物多存在细菌培养阳性,血清IgE呈现高表达状况,两者具有显著相关性,共同参与CRS的发病。

Correlation between Bacteriological Distribution and Serological Markers in Children with Chronic Sinusitis

Abstract:
Objective: To explore the correlation between bacteriological distribution and serological indexes in children with chronic sinusitis (CRS). Methods: From August 2015 to February 2018, 78 children with CRS (group CRS) and 80 healthy children (control group) were selected in our hospital. The nasal secretions of two groups of children were isolated and cultured, and the serum level of IgE in the two groups of children was detected and were given the correlation analysis. Results: The positive rate of bacterial culture in nasal secretion of CRS group were 44.9%, while that of control group were 4.6%, the CRS group were significantly higher than that of control group (P<0.05). In the CRS group, there were 50 strains of bacteria were isolated, included 16 strains of Staphylococcus epidermidis, 10 strains of Enterobacter aerogenes, 6 strains of Staphylococcus pasteuriae, 5 strains of Staphylococcus saprophytic, 5 strains of Aerococus viridans and 8 other strains. The positive rate of serum IgE in CRS group were 71.8%, which were 1.3% in the control group, and the CRS group were significantly higher than in the control group (P<0.05). Spearman correlation analysis showed positive correlation between positive correlation of bacterial culture of sinus secretions and positive serum IgE in CRS group (P<0.05), and no significant correlation in children of control group (P>0.05). Conclusion: There are more bacterial cultures in the sinus secretions of CRS in children and the high expression of serum IgE, and they have significant correlation and participate in the pathogenesis of CRS.
