益生菌防治儿童支气管哮喘的系统评价

摘要:
目的:系统评价益生菌预防和治疗儿童支气管哮喘的疗效。方法:通过the Cochrane Library、PubMed、EMBase、CNKI、万方数据库及中国生物医学文献数据库(CBM)检索益生菌防治儿童支气管哮喘的相关随机对照试验(RCT),检索时限均为建库至2018年8月,按纳入标准与排除标准筛选文献、提取资料并评价纳入研究的方法学质量,采用RevMan 5.3软件进行Meta分析。结果:共纳入10项RCT,包括828例患儿,试验组412例,对照组416例。试验组患儿给予鼠李糖乳杆菌、格氏乳杆菌、干酪乳杆菌、双歧杆菌、德乳杆菌等,对照组患儿给予安慰剂。Meta分析结果显示,试验组和对照组患儿哮喘症状改善方面比较差异有统计学意义[60.00%(51/85) vs 25.88%(22/85),MD=4.28,95%CI:2.23~8.21, P<0.01]。血清细胞因子方面,试验组与对照组患儿在降低IgE [MD=-18.44,95%CI:-20.03~-16.85,P<0.01]、升高IFN-γ [MD=2.50,95%CI:1.23~3.76,P=0.01]及降低IL-4 [MD=-2.25,95%CI:-3.12~-1.39,P<0.01]水平比较差异有统计学意义。结论:益生菌可改善支气管哮喘患儿的哮喘症状以及相关血清细胞因子水平。

Abstract:
Objective: To systematically review the efficacy of probiotics in prevention and treatment of bronchial asthma in children. Methods: The Cochrane Library, PubMed, EMBase, CNKI, Wanfang database and China Biology Medicine (CBM) were retrieved to collect the randomized controlled trials (RCT) of probiotics in prevention and treatment of bronchial asthma in children. According to the inclusion and exclusion criteria, literature were screened, the data were extracted and the methodological quality of the included literature was evaluated. Meta-analysis was performed by using RevMan 5.3 software. Results: A total of 10 RCT were collected, including 828 children, with 412 cases in the experimental group and 416 cases in the control group. Children in the experimental group were given Lactobacillus rhamnosus, Lactobacillus gasseri, Lactobacillus casei, Bifidobacterium and Lactobacillus delte. And the control group received placebo. Meta-analysis results showed that there was statistically significant difference in improvement of asthma symptoms between the experimental group and the control group (60.00% (51/85) vs 25.88% (22/85), MD=4.28, 95%CI: from 2.23 to 8.21, P<0.01). In terms of serum cytokines, there were statistically significant differences between the experimental group and the control group in decreasing IgE (MD=-18.44, 95%CI: from -20.03 to -16.85, P<0.01), increasing IFN-γ (MD=2.50, 95%CI: from 1.23 to 3.76, P=0.01) and decreasing IL-4 (MD=-2.25, 95%CI: from -3.12 to -1.39, P<0.01). Conclusion: Probiotics can improve asthma symptoms and related serum cytokines in children with bronchial asthma.
