沙丁胺醇、异丙托溴胺联合硫酸镁治疗儿童支气管哮喘急性发作疗效观察

摘要:
目的:探究沙丁胺醇、异丙托溴胺联合硫酸镁治疗支气管哮喘急性发作儿童的疗效。方法:选取2018年1月至2019年2月在我院接受治疗的106例支气管哮喘急性发作儿童,采用随机数字表法分为对照组与观察组各53例,对照组采用异丙托溴胺和沙丁胺醇联合雾化吸入治疗,观察组采用异丙托溴胺、沙丁胺醇雾化吸入同时静脉推注硫酸镁,观察两组患儿临床疗效、肺功能指标、血清学指标和不良反应。结果:观察组总有效率高于对照组(P<0.05)。观察组患儿哮鸣音、气促、咳嗽等症状消失时间及住院时间均较对照组短(P<0.05)。治疗后,两组患儿第一秒用力呼气量(FEV1)和最大呼气峰流速(PEF)水平均显著高于治疗前,且观察组各指标水平改善更明显(P<0.05)。治疗后,两组患儿免疫球蛋白E(IgE)水平较治疗前显著降低,且观察组改善更明显(P<0.05)。两组患儿不良反应总发生率比较差异无统计学意义(P>0.05)。结论:沙丁胺醇、异丙托溴胺联合硫酸镁治疗儿童支气管哮喘急性发作显著优于沙丁胺醇和异丙托溴胺,能够提高临床疗效、缩短临床症状改善时间、降低血清IgE水平、改善肺功能。

Abstract:
Objective: To probe into the efficacy of salbutamol and ipratropium bromide combined with magnesium sulfate in the treatment of children with acute attack of bronchial asthma. Methods: Totally 106 children with acute attack of bronchial asthma admitted into our hospital from Mar. 2018 to Feb. 2019 were extracted to be divided into the control group and the observation group via the random number table, with 53 cases in each group. The control group was treated with aerosol inhalation of ipratropium bromide combined with salbutamol, while the observation group received aerosol inhalation of ipratropium bromide combined with salbutamol and intravenous injection of magnesium sulfate. The clinical efficacy, pulmonary function index, serological index and adverse drug reactions of two groups were observed. Results: The total effective rate of the observation group was higher than that of the control group (P<0.05). The disappearance time of wheezing, shortness of breath, cough and other symptoms, the length of stay in the observation group were shorter than those in the control group (P<0.05). After treatment, the first-second forced expiratory volume (FEV1) and maximum expiratory peak flow rate (PEF) in two groups were significantly higher than those before treatment, and the improvement of all indicators in the observation group was more significant (P<0.05). After treatment, the levels of immunoglobulin E (IgE) in two groups were significantly lower than those before treatment, and the reduction in the observation group was more significant (P<0.05). There was no significant difference in the incidence of adverse drug reactions between two groups (P>0.05). Conclusion: Salbutamol, ipratropium bromide combined with magnesium sulfate in the treatment of acute attack of bronchial asthma in children is significantly better than salbutamol combined with ipratropium bromide, which can improve the clinical efficacy, shorten the improvement time of clinical symptoms, reduce the levels of IgE and improve patients’ pulmonary function.
