| 目的:规范临床用药行为,逐步实现儿科药学服务全覆盖。方法:利用医院信息系统收集2018年6月至2019年6月上海健康医学院附属嘉定区中心医院儿科医嘱,根据《医院处方点评管理规范(试行)》点评标准进行全样本医嘱点评,采用帕累托图分析不合理医嘱的影响因素,采用德尔菲法拟定相应儿科药学服务方案。结果:收集的67 842条儿科医嘱中,不合理率5.03%,其中用法用量不适宜、适应证不适宜、开具处方未写临床诊断或临床诊断书写不全等为主要因素。最终拟定以“组织结构—服务过程—服务结果”为主要维度的药学服务方案。结论:儿科药学服务方案是逐步实现儿科药学服务全覆盖的基础,其拟定涉及人员配合、项目优化、质控管理等诸多因素,临床药师具有主动服务意识,才能更好地服务患儿 |

| Pediatric Prescription Review and Pharmaceutical Care for Children in General Hospital |

| Objective: To standardize the behavior of clinical medication and realize the full coverage of pediatric pharmaceutical care. Methods: Medical orders of pediatrics in Jiading Central Hospital, Shanghai University of Medicine & Health Sciences from Jun. 2018 to Jun. 2019 were collected by using the hospital information system. Full-sample medical orders were reviewed based on the Hospital Prescription Review Management Standards (Trial). Pareto diagram was used to analyze the influencing factors of irrational medical orders, and Delphi method was used to formulate corresponding pediatric pharmaceutical care regimen. Results: Among the 67,842 pediatric medical orders, the irrational rate was 5.03%, and the main factors were inappropriate usage and dosage, improper indications, prescriptions without clinical diagnosis or incomplete clinical diagnosis. Eventually, the pharmaceutical care regimen with “organizational structure-service process-service result” as the main dimension was developed. Conclusion: Pediatric pharmaceutical care program is the basis for realizing full coverage of pediatric pharmaceutical care, and its formulation includes many factors such as personnel coordination, project optimization, quality control and management. Only with active service consciousness can the clinical pharmacists serve children better. |
