益生菌治疗儿童功能性腹痛的Meta分析

摘要:
目的:评价益生菌治疗儿童功能性腹痛(FAP)的临床疗效。方法:采用主题词与自由词相结合的方式在PubMed、中国知网、万方数据知识服务平台、维普数据库中检索国内外开展的关于益生菌治疗儿童FAP的临床随机对照试验(RCT),检索时间均为建库至2021年3月1日。使用Rev Man 5.3和Stata1 4.0软件进行Meta分析。结果:最终纳入11篇RCT文献(益生菌组625例,对照组558例)。Meta分析显示,益生菌治疗儿童FAP在缓解腹痛程度[SMD=-0.33,95%CI(-0.64,-0.02),P<0.05]、临床治愈率[OR=2.37,95%CI(1.36,4.13),P<0.05]方面优于安慰剂;降低腹痛发生频率、腹痛持续时间等方面益生菌组与对照组比较差异无统计学意义。结论:益生菌治疗儿童功能性腹痛,在临床治愈率及降低腹痛程度方面优于安慰剂,但在降低腹痛发生频率及持续时间等方面和安慰剂比较差异无统计学意义,提示可能需要开展多中心随机双盲对照试验,明确益生菌的种类、剂量和疗程,以提供更有力的证据支撑

Meta-Analysis of Probiotics in the Treatment of Functional Abdominal Pain in Children

Abstract:
Objective: To evaluate the clinical efficacy of probiotics in the treatment of functional abdominal pain (FAP) in children. Methods: Clinical randomized controlled trials (RCT) on probiotics in the treatment of FAP in children performed at home and abroad were searched in PubMed, CNKI, Wanfang and VIP database by the combination of subject terms and free terms. The retrieval time was from the establishment of the database to Mar. 1st, 2021. Meat-analysis was performed by using RevMan 5.3 and Stata 14.0 software. Results: Eleven RCT (625 cases in the probiotic group and 558 cases in the control group) were eventually included. Meta-analysis showed that probiotics in the treatment of children with FAP was superior to placebo in relieving abdominal pain (SMD=-0.33, 95%CI from -0.64 to -0.02, P<0.05) and clinical cure rate (OR=2.37, 95%CI from 1.36 to 4.13, P<0.05). There was no statistically significant difference between the probiotic group and control group in terms of decreasing the frequency and duration of abdominal pain. Conclusion: Probiotics in the treatment of children with FAP is superior to placebo in terms of clinical cure rate and relieving abdominal pain. Yet there is no significant difference in terms of decreasing frequency and duration of abdominal pain compared with placebo, suggesting that the multi-center randomized double-blind controlled trial may be needed to clarify the category, dose and duration of probiotics to provide stronger evidence.
