| 目的:建立基于加权逼近理想解排序(TOPSIS)法的急性淋巴细胞白血病(ALL)患儿大剂量甲氨蝶呤(HDMTX)药物利
用评价(DUE)标准,促进临床合理用药。 方法:以甲氨蝶呤(MTX)药品说明书为依据,参考 HDMTX 临床使用相关指南和文献,
建立基于加权 TOPSIS 法的 ALL 患儿 HDMTX 的 DUE 标准。 依据 DUE 标准回顾性评价我院 2021-2023 年 HDMTX 使用情况。
结果:150 份病历加权 TOPSIS 法评价显示,相对接近程度(Ci)≤60%的有15 例(10. 00%),60% ~70%的有51 例(34. 00%),>70% ~
80%的有 73 例(48. 67%),≥80%的有 11 例(7. 33%)。 结论:本研究建立的 DUE 标准可用于 HDMTX 在儿童的应用合理性评
价;评价结果显示我院 HDMTX 临床应用存在不合理情况,需进一步加强管控和多学科协作,促进合理用药。 |

| Objective: To establish the drug utilization evaluation ( DUE ) and typical case analysis of high-dose methotrexate
(HDMTX) in children with acute lymphoblastic leukemia (ALL) based on weighted technique for order preference by similarity to ideal
solution (TOPSIS) method, so as to promote the clinical rational application. Methods: Based on drug instructions of methotrexate
(MTX) and by referring to relevant clinical guidelines and literature on the clinical application of HDMTX, DUE criterion for HDMTX
in ALL children based on the weighted TOPSIS method was established. The application of HDMTX in our hospital from 2021 to 2023
was retrospectively evaluated according to the DUE criteria. Results: According to the weighted TOPSIS method evaluation of 150
medical records, the weighted Euclid distance (Ci) was ⩽60% in 15 cases (10%), from 60% to 70% in 51 cases (34. 00%), >70%
to 80% in 73 cases (48. 67%), and ⩾80% in 11 cases (7. 33%). Conclusion: The evaluation criteria of HDMTX in children with
ALL based on weighted TOPSIS method can be used to evaluate the application rationality in children. The evaluation results show that
there are irrational clinical application of HDMTX in our hospital, and it is necessary to further strengthen the control and
multidisciplinary cooperation, and promote the rational medication. |
