小剂量多巴胺辅助治疗新生儿坏死性小肠结肠炎的临床疗效及对炎 症因子水平的影响

摘要:
目的:探讨小剂量多巴胺对新生儿坏死性小肠结肠炎(NEC) 的治疗效果。方法:选择我院收治的NEC 新生儿70 例,按 随机数表法分为对照组和观察组各35 例。对照组采用奥曲肽,观察组加用小剂量多巴胺[2. 5 μg/ (kg·min)]。比较两组患儿 总有效率、临床症状改善情况、炎症因子水平、不良反应发生情况。结果:观察组总有效率(94. 29%)较对照组(77. 14%)明显提 高(P<0. 01);观察组腹胀消退、腹泻缓解、大便改善时间显著短于对照组(P<0. 01);观察组肿瘤坏死因子-α(TNF-α)、C 反应蛋 白(CRP)、白细胞介素-8(IL-8)、IL-6 水平显著低于对照组(P<0. 01);两组患儿出现嗜睡、出汗等不良反应发生率比较差异无统 计学意义(P>0. 05)。结论:对NEC 新生儿采用小剂量多巴胺治疗,可有效减轻机体炎症反应,促进患儿康复,具有较高的临床 应用价值。

Clinical Efficacy of Low-Dose Dopamine in the Adjuvant Treatment of Neonatal Necrotizing Enterocolitisand Effects on Levels of Inflammatory Factors

Abstract:
Objective: To probe into the clinical efficacy of low-dose dopamine in the treatment of patients with neonatal necrotizing enterocolitis (NEC). Methods: Seventy neonates with NEC admitted into our hospital were randomly divided into the control group and observation group, with 35 cases in each group. The control group received octreotide, while the observation group was given low-dose dopamine of 2. 5 μg/ (kg·min). The total effective rate, improvement of clinical symptoms, inflammatory factors and adverse drug reactions were compared between two groups. Results: The total effective rate of observation group (94. 29%) was significantly higher than that of the control group (77. 14%, P<0. 01). The time of abdominal distension, diarrhea relief and stool improvement in the observation group was significantly shorter than that in the control group (P<0. 01). The levels of tumor necrosis factor-α (TNF-α), Creactive protein (CRP), interleukin-8 (IL-8) and IL-6 in observation group were significantly lower than those in control group (P< 0. 01). There was no significant difference in the incidence of sleepiness, sweating and other adverse drug reactions between two groups (P>0. 05). Conclusion: Low-dose dopamine in the treatment of neonates with NEC can effectively reduce the inflammatory response and promote the recovery of the prognosis with higher clinical application value.
