基于属性层次模型的儿童氟康唑药物利用评价

摘要:
目的:建立儿童氟康唑药物利用评价(DUE) 标准,综合评价氟康唑在儿童的应用情况,为临床合理用药提供参考。方 法:依据氟康唑药品说明书、相关指南和专家共识,制订儿童氟康唑DUE 标准。采用回顾性分析方法,以儿童氟康唑DUE 标准 为依据,结合美国西弗吉尼亚大学医学院建立的多疾病风险评分(MDRA)系统,并采用属性层次模型(AHM)对2023 年1-6 月 安徽省儿童医院164 例应用氟康唑的出院患儿病历资料进行分析及合理性评价。结果:根据建立的DUE 标准,儿童氟康唑临 床应用不合理主要表现为超适应证用药、用法用量不适宜和给药后无疗效监测。应用AHM 病例得分( MRS) ≥90 分的63 例 (38. 41%),80 分≤MRS<90 分62 例(37. 80%),60 分≤MRS<80 分36 例(21. 95%),MRS<60 分3 例(1. 83%)。结论:该院氟康 唑临床应用存在部分不合理问题,需加强监督管理,以促进儿童合理用药。基于AHM 建立的儿童氟康唑DUE 标准模型简单实 用,评价结果系统且直观,是一种有效的评价方法。

Abstract:
Objective: To establish the drug use evaluation (DUE) criteria for fluconazole in children, and perform the comprehensive evaluation on rationality of clinical practice of fluconazole, so as to provide basis for clinical rational drug use. Methods: DUE criteria for fluconazole in children was formulated according to drug instructions, relevant guidelines and expert consensus of fluconazole. Retrospective analysis was performed based on the DUE criteria for fluconazole in children, combined with multi-disease risk assessment (MDRA) program established by the West Virginia University School of Medicine, the attribute hierarchy model (AHM) was used to analyze and evaluate the rationality of 164 discharged children treated with fluconazole from Jan. to Jun. 2023 in Anhui Children’s Hospital. Results: Based on the established DUE criteria, the main problems related to the irrational clinical application of fluconazole included off-label indications, inappropriate usage and dosage, and no efficacy monitoring after administration. There were 63 cases (38. 41%) with AHM scores (MRS) ⩾90, 62 cases (37. 80%) with 80⩽MRS<90, 36 cases (21. 95%) with 60⩽MRS<80, and 3 cases (1. 83%) with MRS<60. Conclusion: There are some irrational problems in the clinical application of fluconazole in our hospital, it is necessary to strengthen the management and supervision to promote rational drug use in children. The DUE criteria for fluconazole in children based on AHM model is simple and practical, and the evaluation results are systematic and intuitive.
