注射用头孢曲松钠致儿童胆囊假性结石分析及机制探讨

摘要:
目的:分析注射用头孢曲松钠致儿童胆囊假性结石的发生情况和特点,为儿童安全用药提供参考。方法:回顾性分析 2021 年1 月至2024 年5 月空军军医大学第一附属医院收治的10 例使用注射用头孢曲松钠引起儿童胆囊假性结石患儿病例资 料。结果:本研究共纳入了10 例患儿,其中男6 例(60%),女4 例(40%);年龄17~119 个月;注射用头孢曲松钠日剂量0. 9~ 3. 0 g;给药剂量56. 18~115. 61 mg/ (kg·d);用药疗程3~12 d;用药至胆囊假性结石出现时间2~10 d。结论:注射用头孢曲松 钠致儿童胆囊假性结石的发生率高于成人,大多数患儿无症状,胆囊泥沙样结石较多,影响因素包含药物因素和机体因素,停药 后在2~63 d 内自行消失,因此在使用注射用头孢曲松钠过程中应注意识别高危因素,避免出现此类不良反应。

Analysis on Biliary Pseudolithiasis in Children Induced by Ceftriaxone Sodium for Injection and Discussionon the Mechanism

Abstract:
Objective: To analyze the incidence and characteristics of biliary pseudolithiasis in children induced by ceftriaxone sodium for injection, and to provide reference for safe medication in children. Methods: Data of 10 children with biliary pseudolithiasis induced by ceftriaxone sodium for injection admitted into The First Affiliated Hospital of Air Force Military Medical University from Jan. 2021 to May 2024 were retrospectively analyzed. Results: A total of 10 children were enrolled, including 6 males (60%) and 4 females (40%), aged from 17 to 119 months. The daily dose of ceftriaxone sodium for injection was from 0. 9 to 3. 0 g, the dosage given was from 56. 18 to 115. 61 mg/ (kg·d), with the treatment duration from 3 to 12 d. The time from medication to the appearance of biliary pseudolithiasis in the gallbladder was from 2 to 10 d. Conclusion: The incidence of biliary pseudolithiasis in children induced by ceftriaxone sodium for injection is higher in children than that in adults. Most cases are asymptomatic, with sludge-like stones being the predominant presentation. Risk factors include both drug-related and patient-specific factors. Discontinuation of medication leads to spontaneous resolution of stones within 2 to 63 d. Therefore, identifying high-risk factors during the use of ceftriaxone sodium for injection is crucial to avoid such adverse drug reactions.
