| 目的:评估硫酸镁注射液联合西地那非治疗新生儿持续性肺动脉高压(PPHN)的有效性及安全性。方法:利用计算机检
索PubMed、EMBase、the Cochrane Libriby、Web of Science、中国知网、万方等数据库收录的关于硫酸镁注射液联合西地那非治疗
PPHN 的随机对照试验,应用RevMan 5. 3 软件对所得数据进行Meta 分析。结果:共纳入7 项随机对照研究,包括559 例PPHN
患儿。Meta 分析结果显示,与单用西地那非相比,静脉用硫酸镁联合西地那非可显著降低PPHN 患儿肺动脉收缩压[ MD =
-6. 43,95%CI(-7. 07,-5. 80),P<0. 001]和动脉血二氧化碳分压[MD=-6. 54,95%CI( -9. 42,-3. 66),P<0. 001],提高治疗有
效率[RR=1. 17,95%CI(1. 07,1. 29),P<0. 001],使动脉血氧分压明显增高[ MD= 7. 13,95%CI(3. 72,10. 53),P<0. 001],但两
种治疗方式对体循环收缩压作用未发现明显差异[MD=-6. 11,95%CI(-16. 37,4. 15),P =0. 24]。结论:与单用西地那非相比,
硫酸镁注射液联合西地那非治疗PPHN 具有更好的效果,显著减轻肺动脉压力,改善氧合功能,具有较好的安全性。 |

| Meta-Analysis on Efficacy of Magnesium Sulfate Injection Combined with Sildenafil in the Treatment ofNeonatal Pulmonary Arterial Hypertension |

| Objective: To evaluate the efficacy and safety of magnesium sulfate injection combined with sildenafil in the treatment of
neonatal persistent pulmonary hypertension (PPHN). Methods: PubMed, EMBase, the Cochrane Library, Web of Science, CNKI and
Wanfang databases were retrieved to collect randomized controlled trials of magnesium sulfate injection combined with sildenafil in the
treatment of neonatal PPHN. The data obtained were analyzed through Meta-analysis by using RevMan 5. 3 software. Results: Seven
randomized controlled trials including 559 children with PPHN were included. Meta-analysis indicated that compared with sildenafil
alone, magnesium sulfate injection combined with sildenafil could significantly reduce the pulmonary artery systolic pressure (MD =
-6. 43, 95% CI -7. 07 to -5. 80, P<0. 001) and arterial partial pressure of carbon dioxide (MD=-6. 54, 95% CI -9. 42 to -3. 66,
P<0. 001), increase the treatment effective rate (RR=1. 17, 95% CI 1. 07 to 1. 29, P<0. 001) and arterial partial pressure of oxygen
(MD=7. 13, 95% CI 3. 72 to 10. 53, P<0. 001) in children with PPHN. However, no significant differences were observed regarding
the impact on systemic arterial pressure between two treatments (MD = -6. 11, 95% CI -16. 37 to 4. 15, P = 0. 24). Conclusion:
Compared with sildenafil alone, the efficacy of magnesium sulfate injection combined with sildenafil in the treatment of PPHN is
significant, which can significantly reduce pulmonary artery pressure, and improve oxygenation function with higher safety. |
