肺炎支原体诱发的皮疹黏膜炎1 例并文献复习

摘要:
本研究通过1 例典型病例分析并结合文献回顾,探讨肺炎支原体(MP)诱发的皮疹黏膜炎(MIRM)的临床特征、诊断要 点及治疗策略。患儿为8 岁男孩,以发热、咳嗽起病,后出现眼结膜炎、口腔黏膜糜烂、外生殖器充血及散在靶形样皮疹,符合 MIRM 的经典型表现。实验室检查提示MP 感染,支气管镜及肺泡灌洗液靶向下一代测序进一步确认MP 为致病病原。鉴于患 儿皮疹出现时间与感染进程相符、嗜酸性粒细胞正常、既往无药物过敏史,结合皮损特征,最终排除药物性皮疹,明确诊断为 MIRM。治疗上采用红霉素联合糖皮质激素等多模式综合治疗,并进行局部黏膜护理,病情逐步好转,出院后随访无复发。该病 例提示临床医生在处理肺炎支原体感染伴皮疹黏膜损害时,应高度警惕MIRM 的可能,重视与药物性反应的鉴别,并早期予以 规范治疗以改善预后。

Mycoplasma Pneumoniae-Induced Rash and Mucositis: a Case Report and Literature Review

Abstract:
Objective: To explore the clinical characteristics, diagnostic key points and treatment strategies of Mycoplasma pneumoniaeinduced rash and mucositis (MIRM) through a typical case analysis and literature review. The patient was an 8-year-old male child who initially presented with fever and cough, followed by the development of conjunctivitis, oral mucosal erosion, genital congestion, and scattered target-like skin lesions, consistent with the classic presentation of MIRM. Laboratory tests confirmed MP infection, and bronchoscopy with targeted next-generation sequencing of bronchoalveolar lavage fluid further confirmed MP as the causative pathogen. Given the timing of the skin lesions in relation to the infection, normal eosinophil counts, the absence of a drug allergy history, and the characteristic skin lesions, drug-induced rash was excluded, and a diagnosis of MIRM was confirmed. The child was treated with a combination of erythromycin and corticosteroids, along with local mucosal care, and showed gradual improvement. The child was discharged after treatment, and follow-up after one month revealed no recurrence. This case serves as an important reminder to clinicians that in patients presenting with MP infection accompanied by rash and mucosal lesions, it is crucial to maintain a high vigilance for MIRM, differentiate it from drug-induced reactions, and ensure early, standard intervention, which is essential for a favorable prognosis.
