| 目的:探讨中药熏蒸联合重组人表皮生长因子( rhEGF) 治疗儿童干眼症( DED) 的疗效及对眼表功能、泪液白细胞介素
(IL)-6、IL-17、肿瘤坏死因子(TNF)-α 的影响。方法:选取唐山市妇幼保健院2023 年1 月至2024 年5 月收治的116 例DED 患
儿,采用随机数表法分为rhEGF 组和熏蒸组各58 例。rhEGF 组采用rhEGF 滴眼液治疗,熏蒸组在rhEGF 组治疗基础上联合中
药熏蒸。比较两组患儿疗效、中医证候积分、眼表功能、泪液炎症因子、生活质量、不良反应发生情况。结果:熏蒸组患儿治疗总
有效率为96. 55%,高于rhEGF 组的82. 76%(P<0. 05)。熏蒸组中医证候积分、角膜荧光素染色(FLS) 评分、IL-6、IL-17、TNF-α
水平均低于rhEGF 组(P<0. 05)。治疗后,熏蒸组泪膜破裂时间(BUT)、泪液分泌量、各维度生活质量评分均高于rhEGF 组(P<
0. 05)。结论:中药熏蒸联合rhEGF 治疗儿童DED 疗效显著,可改善患儿中医证候,通过下调IL-6、IL-17、TNF-α 表达,抑制炎症
水平,控制疾病进展,减少角膜损伤程度,提高泪膜稳定性,改善泪液分泌功能,提高患儿生活质量,安全性较高。 |

| Objective: To investigate the efficacy of traditional Chinese medicine fumigation combined with recombinant human
epidermal growth factor (rhEGF) in the treatment of children with dry eye disease (DED) and its effects on ocular surface function, tear
fluid interleukin-6 (IL-6), IL-17, and tumor necrosis factor (TNF)-α. Methods: Totally 116 children with DED admitted into
Tangshan Maternal and Child Health Hospital from Jan. 2023 to May 2024 were extracted to be divided into the rhEGF group and
fumigation group via random number table method, with 58 cases in each group. The rhEGF group was treated with rhEGF eye drops,
while the fumigation group received traditional Chinese medicine fumigation on the basis of the rhEGF treatment. The efficacy,
traditional Chinese medicine syndrome score, ocular surface function, tear inflammatory factors, quality of life and adverse drug
reactions were compared between two groups. Results: The total effective rate of fumigation group was 96. 55%, higher than that
82. 76% of rhEGF group (P<0. 05). The traditional Chinese medicine syndrome scores, corneal fluorescein staining (FLS) scores, IL-6,
IL-17 and TNF-α levels in fumigation group were lower than those in rhEGF group (P<0. 05). After treatment, the tear film break-up
time (BUT), tear secretion volume, and quality of life scores in each dimension of fumigation group were higher than those of rhEGF
group (P<0. 05). Conclusion: Traditional Chinese medicine fumigation combined with rhEGF has significant therapeutic effects, which
can improve the traditional Chinese syndrome, reduce the expression of IL-6, IL-17, and TNF-α, thereby inhibiting the inflammatory
level, controlling the disease progression, reducing the degree of corneal damage, improving the stability of the tear film, enhancing the
tear secretion function, and improving the quality of life with higher safety. |
