| 目的:探讨并构建儿科全天候静脉用药集中调配工作模式。 方法:通过静脉用药调配中心(PIVAS)项目规划建设、信息 系统支持、注射类药物调配设计、24 h 工作流程设计等举措,构建一套包含全医嘱审核、全静脉用药种类、全医嘱类型、全时段、 全静脉给药方式、危害药品全注射给药途径 6 项基本内容的 PIVAS“六全”调配模式。 结果:实施“六全”模式后(2022-2024 年)的数据显示,我院 PIVAS 调配的临时医嘱成品占比 22. 40%,静脉推注和静脉泵入成品占比 29. 27%,非静脉滴注危害药品 占比 45. 82%;临时调配间工作量占比 23. 29%。 结论:“六全”模式能满足儿科 PIVAS 全天候运行精细化管理,有效提升调配和 药学服务质量。 |

| Construction and Exploration of the Pediatric “ Six-Comprehensive ” Model for Pharmacy Intravenous Admixture Services Based on Clinical Needs |

| Objective: To explore and construct the round-the-clock model for pediatric pharmacy intravenous admixture. Methods: Through measures such as the construction of the pharmacy intravenous admixture services ( PIVAS) project, information system support, dispensing design of injection drugs, and the 24-h workflow design, the PIVAS “ six-comprehensive ” dispensing model including comprehensive review of medical orders, comprehensive intravenous medication types, comprehensive medical orders types, round-the-clock, comprehensive intravenous administration route, and comprehensive administration route of harmful drugs for injection was constructed. Results: The data from 2022 to 2024 following the implementation of the “six-comprehensive” model showed that the proportion of temporary medical orders in the finished products prepared by PIVAS was 22. 40%, the proportion of finished products for intravenous injection and infusion was 29. 27%, and the proportion of non-intravenous drip hazardous drugs was 45. 82%; the workload of the temporary preparation room accounted for 23. 29%. Conclusion: The “six-comprehensive” model can ensure the round-the-clock operation of pediatric PIVAS with refined management, effectively improving the quality of dispensing and pharmaceutical services. |
