| 目的:探讨A 型肉毒毒素(BTX-A)联合任务导向性训练(TOT)治疗儿童痉挛型脑性瘫痪(SCP)的效果。方法:选取邢台
市中心医院2022 年12 月-2024 年12 月收治的SCP 患儿126 例,按随机信封法分为BTX-A 组、TOT 组和联合组各42 例。BTX-A
组使用BTX-A 注射治疗,TOT 组使用TOT 方案,联合组使用BTX-A 注射联合TOT 方案治疗。观察比较各组患儿疗效、关节被
动活动度、平衡能力、痉挛程度、运动能力、日常生活能力等。结果:联合组总有效率高于BTX-A 组和TOT 组(P<0. 05);治疗后
三组患儿足背屈角、踝关节改良Ashworth 痉挛量表( MAS) 评分、小腿三头肌综合痉挛量表( CSS) 评分均降低,Berg 平衡量表
(BBS)、粗大运动功能评价量表(GMFM-88)、Fugl-Meyer 运动功能评定表(FMA) 评分均升高,且联合组变化更明显(P<0. 05)。
结论:BTX-A 联合TOT 治疗SCP 效果明显,可显著改善患儿尖足症状、运动功能和日常生活活动能力。 |

| Efficacy of Botulinum Toxin Type A Injection Combined with Task-Oriented Training in Children with SpasticCerebral Palsy |

| Objective: To observe the effects of botulinum toxin type A (BTX-A) combined with task-oriented training (TOT) in the
treatment of spastic cerebral palsy (SCP). Methods: From Dec. 2022 to Dec. 2024, totally 126 children with SCP were extracted to be
divided into BTX-A group, TOT group, and combined group via the sealed envelope randomization, with 42 patients in each group. The
BTX-A group was treated with BTX-A injection, the TOT group was given TOT protocol, and the combined group received BTX-A
injection combined with TOT protocol. The efficacy, passive joint range of motion, balance ability, spasticity degree, motor ability and
activities of daily living were observed and compared. Results: The total effective rate of combined group was higher than that of BTX-A
group and TOT group (P<0. 05). After treatment, the dorsiflexion angle of feet, the Modified Ashworth Scale (MAS) score of ankle
joint, and the Composite Spasticity Scale (CSS) score of gastrocnemius muscle in three groups decreased, while the Berg Balance Scale
(BBS), the Gross Motor Function Measure (GMFM-88), and the Fugl-Meyer Assessment (FMA) scores increased, the changes in the
combined group were more significant (P <0. 05). Conclusion: BTX-A combined with TOT demonstrates significant efficacy in the
treatment of SCP, and can significantly improve the symptoms, motor function, and activities of daily living in children. |
