基于肺炎支原体肺炎患儿血清Krüppel 样因子5、可溶性晚期糖基化终末 产物受体、血管生成素-1 水平建立转归预测模型及早期药物治疗策略

摘要:
目的:探讨基于肺炎支原体肺炎(MPP)患儿血清Krüpple 样因子5(KLF5)、可溶性晚期糖基化终末产物受体(sRAGE)、血 管生成素-1(Ang-1)水平建立转归预测模型及早期干预策略。方法:选取2022 年6 月至2024 年6 月我院收治的MPP 患儿220 例 为MPP 组,选取100 例同期体检健康的儿童作为对照组。结果:MPP 组患儿血清Ang-1 水平低于对照组,KLF5、sRAGE 水平高于 对照组(P<0. 01)。MPP 组重症组血清Ang-1 水平低于轻症组,KLF5、sRAGE 水平高于轻症组(P<0. 01)。血清KLF5、sRAGE 水平 与MPP 严重程度及预后呈正相关,血清Ang-1 水平与MPP 严重程度及预后呈负相关(P<0. 05)。血清KLF5、sRAGE、Ang-1 联合 预测MPP 患儿预后的AUC 优于各自单独预测(Z联合- KLF5 =5. 642、Z联合- sRAGE =4. 867、Z联合- Ang-1 =3. 923,P<0. 05)。结论:MPP 患儿血 清KLF5、sRAGE 及Ang-1 水平变化与疾病严重程度及预后相关,三者联合检测对MPP 患儿预后的预测价值较高。

Establishment of Prognostic Model and Early Intervention Strategy Based on Serum Krüpple-Like Factor 5,Soluble Receptor of Advanced Glycation End Products, and Angiopoietin-1 Levels in Children with MycoplasmaPneumoniae Pneumonia

Abstract:
Objective: To probe into the establishment of prognostic prediction model and early intervention strategy based on serum Krüpple-like factor 5 (KLF5), soluble receptor of advanced glycation end products (sRAGE), and angiopoietin-1 (Ang-1) levels in children with Mycoplasma pneumoniae pneumonia (MPP). Methods: A total of 220 children with MPP admitted to our hospital from Jun. 2022 to Jun. 2024 were extracted as the MPP group, and 100 healthy children who underwent physical examinations during the same period were selected as the control group. Results: The serum Ang-1 level in the MPP group was lower than that in the control group, while the KLF5 and sRAGE levels were higher than those in the control group (P<0. 01). The serum Ang-1 level in the MPP severe group was lower than that in the mild group, while the KLF5 and sRAGE levels were higher than those in the mild group (P< 0. 01). The levels of serum KLF5 and sRAGE were positively correlated with the severity and prognosis of MPP, while the level of serum Ang-1 was negatively correlated with the severity and prognosis of MPP (P <0. 05). The AUC of predicting the prognosis of children with MPP by combination of serum KLF5, sRAGE, and Ang-1 was better than separate prediction (Zcombination-KLF5 = 5. 642, Zcombination-sRAGE =4. 867, Zcombination-Ang-1 =3. 923, P<0. 05). Conclusion: Changes in serum KLF5, sRAGE, and Ang-1 levels in children with MPP are correlated with disease severity and prognosis. The combined detection of KLF5, sRAGE4, and Ang-1 has a high predictive value for the prognosis of children with MPP.
