| 目的:探讨肠道病毒71 型(EV71)重症手足口病(HFMD)患儿肠道菌群构成、丰度、免疫功能和临床特征与益生菌干预
临床转归的关系。方法:选择2023 年1 月至2024 年6 月于岳阳市妇幼保健院就诊的80 例重症HFMD 患儿作为观察组,另选择
我院同期就诊的80 例轻症HFMD 患儿作为对照组。比较两组患儿肠道菌群的构成和丰度,分析其中存在数量差异的肠道菌群
与免疫功能指标及患儿临床转归的关系。结果:与对照组比较,观察组肠道菌群中双歧杆菌科、瘤胃球菌科与梭菌科的相对丰
度均较低(均P<0. 05)。双歧杆菌科、瘤胃球菌科与梭菌科的相对丰度与血清IgA、IgM 和IgG 水平均呈正相关(均P<0. 05),且
上述3 种细菌相对丰度降低是重症HFMD 患儿临床转归不良的危险因素(P<0. 05)。结论:EV71 型重症HFMD 患儿肠道中双
歧杆菌科、瘤胃球菌科与梭菌科的细菌相对丰度均减少,且上述菌群相对丰度与患儿的免疫功能和临床转归均密切相关。 |

| Abstract: |
| Objective: To explore the composition and abundance of intestinal flora in children with severe hand-foot-mouth disease
(HFMD) caused by EV71 and its correlation with immunity and intervention effect of probiotics. Methods: A total of 80 children with
severe HFMD from yueyang Maternal and Child Health-Care Hospital from Jan. 2023 to Jun. 2024 were selected as the observation
group, and another 80 children with HFMD were selected as the control group. The composition and abundance of intestinal flora were
compared between two groups, and the correlation between gut microbiota with significant quantity differences and immune function
indicators, as well as clinical outcomes of children was analyzed. Results: Compared with control group, the relative abundances of
Bifidobacteria, Ruminococcus and Clostridium in the gut microbiota of observation group was lower (P<0. 05). The relative abundances
of Bifidobacteria, Ruminococcus and Clostridium were positively correlated with levels of serum IgA, IgM and IgG (P<0. 05). Decreased
relative abundances of Bifidobacteria, Ruminococcus and Clostridium were risk factors of poor clinical outcomes in children with severe
HFMD (P<0. 05). Conclusion: Quantities of Bifidobacteria, Ruminococcus and Clostridium in the intestines of children with severe
HFMD caused by EV71 are reduced, and quantities of these bacterial groups are closely related to the immune function and clinical
outcomes of children. |
