细菌溶解产物辅助治疗咳嗽变异性哮喘患儿的疗效评价

摘要:
目的:探讨细菌溶解产物辅助治疗咳嗽变异性哮喘(CVA)患儿的疗效及其对外周血辅助性T 细胞1(Th1) / 辅助性T 细 胞2(Th2)细胞因子水平的影响。方法:选取2023-2024 年武警上海市总队医院收治的110 例CVA 患儿,根据治疗方案分为对 照组(采用常规对症治疗) 和观察组(加用细菌溶解产物辅助治疗),每组各55 例。比较两组患儿临床疗效、气道重塑指标、 Th1/ Th2 细胞因子、肺功能、生活质量及不良反应发生情况。结果:治疗2 个月后,观察组临床总有效率高于对照组,哮喘儿童 生存质量问卷(PAQLQ)各维度评分高于对照组(P<0. 05)。治疗2 个月后,观察组气道壁总面积和气道壁厚度、白细胞介素-4 (IL-4)水平低于对照组(P<0. 05),观察组血清干扰素-γ(IFN-γ)、IFN-γ/ IL-4 值、最大呼气峰流速、第一秒用力呼气容积、第一秒 用力呼气容积/ 用力肺活量指标高于对照组( P < 0. 05)。治疗期间, 两组患儿不良反应发生率比较差异无统计学意义 (P>0. 05)。在多因素校正后,观察组临床疗效仍显著优于对照组。结论:细菌溶解产物辅助治疗CVA 患儿疗效确切,可有效 调节Th1/ Th2 细胞因子平衡,抑制气道重塑,改善肺功能与生活质量,且安全性良好。

Abstract:
Objective: To explore the efficacy of bacterial dissolution product in the adjuvant treatment of children with cough variant asthma (CVA) and its effects on cytokine levels of peripheral blood helper T cell 1 (Th1) / T cell 2 (Th2). Methods: A total of 110 children with CVA admitted into Shanghai Armed Police General Hospital from 2023 to 2024 were extracted to be divided into control group (conventional symptomatic treatment) and observation group (with the addition of bacterial dissolution product for adjuvant treatment) according to different treatment regimens, with 55 cases in each group. The clinical efficacy, airway remodeling index, Th1/ Th2 cytokines, lung function, quality of life and adverse drug reactions were compared between two groups. Results: After 2 months of treatment, the total clinical effective rate of observation group was higher than that of control group, and the scores of each dimension of the Pediatric Asthma Quality of Life Questionnaire (PAQLQ) in the asthma children were higher than those of control group (P<0. 05). After 2 months of treatment, the total airway wall area, airway wall thickness parameters, interleukin-4 (IL-4) in observation group were lower than those in control group (P<0. 05). The serum interferon-γ (IFN-γ), IFN-γ/ IL-4, peak expiratory flow rate, forced expiratory volume at one second, and forced expiratory volume at one second/ forced vital capacity in observation group were higher than than those in control group (P<0. 05). There was no significant difference in the incidence of adverse drug reactions between two groups during treatment (P >0. 05). After adjusting the multiple factors, the clinical efficacy of observation group was significantly better than that of control group. Conclusion: Bacterial dissolution product have a definite efficacy on children with CVA, which can effectively regulate Th1/ Th2 cytokines balance, inhibit airway remodeling, improve lung function and quality of life, and have good safety.
