基于H2A 组蛋白家族成员X、非SMC 缩合蛋白Ⅰ复合物亚基G、异常纺锤体样小头畸形相关基因表达水平评估神经母细胞瘤患儿预后及早期防治策略

摘要:
目的:探讨神经母细胞瘤患儿组织H2A 组蛋白家族成员X(H2AFX)、非SMC 缩合蛋白Ⅰ复合物亚基G(NCAPG)、异常纺锤体样小头畸形相关基因(ASPM)表达水平及其与临床特征、预后的关系。方法:选取2017 年12 月至2021 年12 月我院收治的93 例神经母细胞瘤患儿,采用实时荧光定量聚合酶链式反应(qRT-PCR)法检测患儿H2AFX、NCAPG、ASPM 在病灶组织及癌旁组织中的表达情况;Kaplan-Meier 生存分析探讨H2AFX、NCAPG、ASPM 表达与神经母细胞瘤患儿预后的关系;COX 分析神经母细胞瘤患儿预后影响因素。结果:病灶组织H2AFX、NCAPG、ASPM 水平高于癌旁组织( P<0. 05)。H2AFX、NCAPG、ASPM表达与MYCN 基因状态、性别、病灶部位、年龄无关,与危险度、国际神经母细胞瘤分期系统( INSS) 分期、肿瘤转移有关( P<0. 05)。死亡组H2AFX、NCAPG、ASPM 水平高于生存组(P<0. 05)。H2AFX≥1. 73、NCAPG≥1. 77、ASPM≥1. 92 表达的患儿预后3 年生存率低于H2AFX<1. 73、NCAPG<1. 77、ASPM<1. 92 表达的患儿( P<0. 05)。多因素COX 回归分析表明,高危、INSS 分期 处于Ⅲ~Ⅳ-S 期、肿瘤发生转移及H2AFX、NCAPG、ASPM 水平均为神经母细胞瘤患儿危险因素(P<0. 05)。结论:神经母细胞瘤患儿组织H2AFX、NCAPG、ASPM 表达水平升高,三者与患儿临床特征及预后生存率密切相关。

Prediction of Prognosis and Early Intervention and Prevention Strategies for Children with Neuroblastoma Based on Expression Levels of H2A Histone Family Member X, Non-SMC Condensin Ⅰ Complex Subunit G, and Abnormal Spindle-Like Microcephaly-Associated Gene

Abstract:
Objective: To investigate the expression levels of H2A histone family member X (H2AFX), non-SMC condensin Ⅰ complex subunit G (NCAPG), and abnormal spindle-like microcephaly-associated gene (ASPM) in the tissues of children with neuroblastoma, and to explore the correlation with clinical characteristics and prognosis. Methods: From Dec. 2017 to Dec. 2021, totally 93 children with neuroblastoma in our hospital were enrolled. The expression levels of H2AFX, NCAPG and ASPM in the lesion tissues and adjacent tissues were detected by quantitative real-time polymerase chain reaction (qRT-PCR). Kaplan-Meier survival was applied to discuss the correlation between expression of H2AFX, NCAPG, ASPM and prognosis of neuroblastoma. COX method was used to explore the prognostic factors of children with neuroblastoma. Results: The expression levels of H2AFX, NCAPG, and ASPM in lesion tissues were significantly higher than those in the adjacent tissues (P<0. 05). The expression levels of H2AFX, NCAPG, and ASPM were not related to MYCN gene status, gender, lesion location, and age of children, but were related to risk, International Neuroblastoma Staging System (INSS) staging, and tumor metastasis. The expression levels of H2AFX, NCAPG and ASPM in death group were higher than those in survival group (P<0. 05). Multivariate COX regression analysis showed that high-risk status, INSS stage at Ⅲ to Ⅳ-S, tumor metastasis, expression levels of H2AFX, NCAPG, and ASPM were risk factors for children with neuroblastoma (P<0. 05). Conclusion: The expression levels of H2AFX, NCAPG, and ASPM in tissues of children with neuroblastoma are elevated, thethree are closely related to clinical characteristics and prognosis survival rate of children.
