| 目的:分享临床药师参与 1 例超低出生体质量早产儿伏立康唑治疗烟曲霉感染的实践经验。 方法:临床药师深度参与 患儿诊疗过程,结合指南文献制定伏立康唑阶梯式治疗方案,通过血药浓度监测、不良反应防控开展个体化药学监护。 结果:患 儿伏立康唑谷浓度持续在有效窗内,烟曲霉感染控制良好,无肝肾损伤等不良反应,用药安全有效。 结论:伏立康唑用于治疗该 类患儿烟曲霉感染具有一定安全性和有效性;临床药师深度参与临床诊疗可保障用药安全,促进特殊人群抗真菌合理用药。 |

| Objective: To share the practical experience of clinical pharmacists in managing voriconazole therapy for Aspergillus fumigatus infection in an extremely low birth weight infant. Methods: Clinical pharmacists actively participated into the treatment process of the children, formulated a stepwise treatment regimen for voriconazole based on current clinical guidelines and available scientific literature, and carried out individualized pharmaceutical care through blood drug concentration monitoring and prevention and control of adverse drug reactions. Results: Voriconazole trough concentrations were consistently maintained within the therapeutic range, achieving effective control of A. fumigatus infection. No hepatic or renal impairment or other significant adverse drug reactions were observed, indicating a favorable safety and efficacy profile of the treatment. Conclusion: Voriconazole is a safe and effective option for the management of A. fumigatus infection in extremely low birth weight infants. The active involvement of clinical pharmacists can ensure medication safety and promote the rational use of antifungal drugs among special populations. |
