| 两种非侵入性镇静方案缓解儿童术前焦虑的随机双盲研究 |

| 摘要: |
| 目的:比较咪达唑仑口服与右美托咪定喷鼻对 1~6 岁儿童术前焦虑的镇静效果与安全性。 方法:采用随机双盲对照设 计,纳入 1~6 岁择期全麻手术患儿 81 例(ASAⅠ~Ⅱ级),随机分为咪达唑仑口服液组(M 组,n = 41,0. 82 mg / kg)和右美托咪定 喷鼻组(D 组,n = 40,2 μg / kg)。 主要研究指标为镇静成功率(改良 Ramsay 镇静评分≥3a、分离焦虑评分≤2 分且面罩诱导接受 评分≤2 分),次要研究指标包括给药时行为评分、补救镇静率、家长满意度及不良反应。 结果:M 组镇静成功率为 92. 7%,高于 D 组的 70. 0%(P = 0. 009)。 M 组患儿给药依从性更好、面罩诱导接受评分更低、镇静成功分离时间更短(P<0. 01)。 M 组家长 满意度高于 D 组(P = 0. 004)。 D 组发生 1 例窦性心动过缓,而 M 组无药物相关不良事件发生。 结论:与 2 μg / kg 右美托咪定喷 鼻相比,0. 82 mg / kg 咪达唑仑口服液用于缓解小儿术前焦虑镇静成功率更高,且患儿接受度更高。 |

| Abstract: |
| Objective: To compare the sedative efficacy and safety of oral midazolam and intranasal dexmedetomidine for preoperative anxiety in children aged from 1 to 6 years. Methods: Eighty-one children (ASA Ⅰ to Ⅱ) scheduled for elective general anesthesia were randomly divided into the oral midazolam group (group M, n = 41, 0. 82 mg / kg) and intranasal dexmedetomidine group (group D, n = 40, 2 μg / kg). The primary outcome was sedation success rate (modified Ramsay score⩾3a, separation anxiety score ⩽2 points and mask acceptance score ⩽2 points). Secondary outcomes included behavioral score during administration, rescue sedation rate, parental satisfaction and adverse drug reactions. Results: The sedation success rate was 92. 7% in group M, higher than 70. 0% in group D (P=0. 009). Group M had better medication compliance, lower mask acceptance score and shorter time to successful separation (P<0. 01), and higher parental satisfaction (P = 0. 004) than group D. One case of sinus bradycardia was observed in group D, while no drug- related adverse events were found in group M. Conclusion: Compared with intranasal dexmedetomidine (2 μg / kg), oral midazolam (0. 82 mg / kg) has higher sedation success rate and better acceptance in relieving preoperative anxiety in children. |
