早产儿支气管肺发育不良并发肺动脉高压的危险因素分析

摘要:
目的:探讨早产儿支气管肺发育不良(BPD)并发肺动脉高压(PH)的危险因素。方法:回顾性分析202例BPD患儿的临床资料,根据是否并发PH分为并发PH组(n=79)和未并发PH组(n=123),采用单因素和多因素Logistic回归分析法,总结BPD并发PH的危险因素。结果:单因素分析显示,两组在小于胎龄儿(SGA)、呼吸窘迫综合征(RDS)、有血流动力学意义的动脉导管未闭(hsPDA)、败血症、中重度BPD、有创通气≥7 d等指标比较差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,RDS和hsPDA是BPD并发PH的独立危险因素。结论:RDS和hsPDA是BPD并发PH的独立危险因素。对于BPD患儿,临床应早期识别其危险因素并及时干预,尽可能降低并发PH的风险。

Abstract:
Objective: To probe into the risk factors for bronchopulmonary dysplasia (BPD) complicated with pulmonary hypertension (PH) in preterm infants. Methods: Clinical data of 202 children with BPD were retrospectively analyzed, all children were divided into the PH group (n=79) and the non-PH group (n=123) according to whether BPD was complicated with PH. Univariate and multivariate Logistic regression analysis was used to summarize the risk factors of BPD complicated with PH. Results: Univariate analysis showed that there were statistically significant differences between two groups in small gestational age (SGA), respiratory distress syndrome (RDS), haemodynamieally significant patent duetus arteriosus (hsPDA), sepsis, moderate to severe BPD, invasive ventilation ≥7 d and other indicators (P<0.05). Multivariate analysis showed that RDS and hsPDA were independent risk factors for BPD complicated with PH. Conclusion: RDS and hsPDA are independent risk factors for BPD complicated with PH. For children with BPD, clinical risk factors should be identified early, timely intervention should be made to reduce the risks of BPD complicated with PH.
