某院2016-2019年儿童导管相关性尿路感染病原菌分布和耐药性分析

摘要:
目的:为抗菌药物经验性治疗儿童导管相关性尿路感染(CAUTI)提供参考。方法:回顾性收集2016-2019年苏州大学附属儿童医院住院患儿导管相关性尿路感染尿培养分离的病原菌119株,统计分析病原菌分布情况和药敏试验结果。结果:革兰阳性菌86株(72.3%),革兰阴性菌27株(22.7%),真菌6株(5.0%)。常见病原菌依次是粪肠球菌55株(46.2%),屎肠球菌20株(16.8%),大肠埃希菌10株(8.4%)。患儿年龄(31.9±8.8)个月,其中男80例(67.2%)。粪肠球菌和屎肠球菌对万古霉素、利奈唑胺、替考拉宁敏感率均为100%。粪肠球菌对氨苄西林敏感率81.8%,对左氧氟沙星敏感率为96.4%,而屎肠球菌对氨苄西林和左氧氟沙星耐药。结论:儿童导管相关性尿路感染病原菌以粪肠球菌和屎肠球菌为主,定期开展医疗机构病原菌分布及耐药性监测工作,对指导抗菌药物合理应用及医院感染管控十分重要。

Distribution and Drug Resistance of Pathogenic Bacteria in Children with Catheter-Associated Urinary Tract Infection in a Hospital from 2016 to 2019

Abstract:
Objective: To provide reference for the empirical treatment of catheter-associated urinary tract infection (CAUTI) in children with antibiotics. Methods: Totally 119 strains of pathogenic bacteria isolated from urine culture of CAUTI in hospitalized children from Children’s Hospital of Soochow University from 2016 to 2019 were retrospectively collected. Distribution of pathogenic bacteria and results of drug sensitivity test were analyzed. Results: There were 86 strains (72.3%) of Gram-positive bacteria, 27 strains (22.7%) of Gram-negative bacteria, and 6 strains (5.0%) of fungi. The pathogenic bacteria seen in order were Enterococcus faecalis (55 strains, 46.2%), Enterococcus faecalis (20 strains, 16.8%) and Escherichia coli (10 strains, 8.4%). The age of children was (31.9±8.8) months, and 80 cases (67.2%) were males. The sensitivity rates of E. faecalis and E. faecium to vancomycin, linezolid and teicoranin were 100%. The susceptibility rate of E. faecalis to ampicillin was 81.8% and that of levofloxacin was 96.4%, while E. faecium was resistant to ampicillin and levofloxacin. Conclusion: The pathogenic bacteria of CAUTI in children are mainly E. faecalis and E. faecium. Regular monitoring of pathogen distribution and drug resistance in medical institutions is essential to guide the rational application of antibiotics and management of nosocomial infection.
