| 某院儿科药房分诊断单元服务模式的实施成效与持续改进 |

| 目的:评价分诊断单元服务模式在减少医院儿科药房内部调剂差错事件方面的成效。方法:采用回顾性研究方法,对东莞市第五人民医院儿科药房2015年登记的71例内部调剂差错进行分类统计,分析原因,评价分诊断单元服务模式的实施成效,持续改进服务质量。结果:东莞市第五人民医院儿科药房在传统服务模式下(2015年1-6月) 发生内部调剂差错事件53例,在分诊断单元服务模式下(2015年7-12月)发生内部调剂差错事件18例,差错率由0.540‰下降到0.182‰(P<0.01)。儿科药房内部调剂差错事件的原因主要分为人员因素、环境因素、药品因素和工作流程因素4个方面,可从这4个方面着手持续改进服务质量。结论:分诊断单元服务模式在减少儿科药房内部调剂差错事件方面具有良好的应用前景。 |

| Objective: To evaluate the effect of diagnoses-based subunit service on reducing dispensing errors in pediatrics pharmacy. Methods: Retrospective analysis method was done in Fifth People's Hospital of Dongguan in 2015, seventy one cases of pediatric pharmacy dispensing errors were analyzed, the effect of diagnoses-based subunit service was evaluated. Results: After the implementation of diagnoses-based subunit service, dispensing errors dropped to 18 from 53 cases, error rate dropped from 0.540‰ to 0.182‰ (P<0.05). In our hospital the reasons of pediatric pharmacy dispensing error events were mainly divided into internal personnel factors, environmental factors, drug factors and process factors, we can continuously improve service quality from the four aspects. Conclusion: Diagnoses-based subunit service has a bright prospect to reduce pediatric pharmacy dispensing error events. |
