| 目的:探讨苯丙酮尿症(PKU)患儿早期治疗依从性对其预后的影响,并分析提升早期治疗依从性的措施。方法:选取2005年1月至2015年1月十堰市妇幼保健院新生儿筛查分中心筛查确诊的PKU患儿74例为研究对象,回顾性分析其病历资料,按早期治疗依从性是否良好分为依从性良好组和依从性不佳组,比较两组患儿一般资料、家庭情况、身高、体质量及智力变化情况。结果:治疗依从性良好组共48例,治疗依从性不佳组共26 例,两组患儿母亲学历、家庭年收入、确诊时间、是否参与医保比较差异均有统计学意义(P<0.05);两组身高逸中位数、体质量逸中位数比例比较差异无统计学意义(P>0.05),但治疗依从性良好组神经行为发育量表(Gesell发育量表)各行为能区评分均高于治疗依从性不佳组,差异有统计学意义(P<0.05)。结论:PKU患儿早期治疗依从性较差,主要受家庭情况影响,较差的早期治疗依从性可能对患儿智力发育造成不良影响,需注意预防。 |

| Objective: To study the effect of early treatment compliance on the prognosis of children with phenylketonuria (PKU), and to analyze the measures to improve the compliance of early treatment. Methods: Seventy four children with PKU diagnosed by Shiyan Maternal and Child Health Care Hospital Neonatal Screening Center from January 2005 to January 2015 were selected as the subjects, The medical records were retrospectively analyzed. According to early treatment compliance, all children were divided into good
compliance group and poor compliance group, compared the two groups with general information, family status, height, body mass and mental changes. Results: There were 48 cases in good compliance group and 26 cases in poor compliance group, the differences in mother's education backgroud, family annual income, diagnosis time and participation in medical insurance between the two groups were statistically significant (P<0.05), the differences had no statistical significance in weight≥median and height≥median in two groups (P>0.05). The scores of Gesell behavioral scale of behavior function areas in good compliance group were higher than those of the poor compliance group, the differences were statistically significant (P<0.05). Conclusion: The early treatment compliance of PKU children is poor, which is mainly affected by family conditions. Poor early treatment compliance may have adverse effects on children's mental development, and need to pay attention to prevention. |
