新生儿ABO溶血症与O型血母亲抗体效价相关性研究

摘要:
目的:探讨新生儿ABO溶血症(ABO-HDN)与O型血母亲抗体效价的相关性。方法:选择712例O型血母亲所产新生儿,对血型为A型、B型的新生儿做ABO-HDN检测,并取其母亲的静脉血进行抗A/B免疫球蛋白G(IgG)效价检测,分析母亲抗IgG效价与ABO-HDN发生率的关系。结果:本组新生儿共检出111例ABO-HDN阳性患儿,ABO-HDN发生率为15.59%(111/712)。ABO-HDN发生率与母亲抗体IgG效价呈正相关(r=0.96,P<0.01);ABO-HDN患儿黄疸出现时间与母亲抗IgG效价无显著相关性(P>0.05)。母亲抗IgG效价≤1∶64组溶血新生儿达到光疗指征率、1周内贫血发生率及换血率均低于其他两组(P<0.05);母亲抗IgG效价≥1∶256组溶血新生儿1周内贫血发生率及换血率高于母亲抗IgG效价1∶128组(P<0.05)。结论:对O型血母亲所产A型或B型血新生儿,早期行溶血三项及抗体效价检查,可预估新生儿发生溶血的可能性及严重程度。

Abstract:
Objective: To investigate the correlation between the newborn with ABO hemolytic disease (ABO-HDN) and the antibody titer of the mother with type O blood. Methods: Seven hundreds and twelve cases of type O blood mothers of newborns were selected as a research object, the ABO-HDN of newborns with type A blood and type B blood were tested, and the mother's blood were taken for anti-A/B immunoglobulin G (IgG) titer detection, relationship between maternal anti IgG titer and the incidence of ABO-HDN were analyzed. Results: A total of 111 neonates with ABO-HDN positive were detected in this group, and the incidence of ABO-HDN was 15.59% (111/712). The incidence of ABO-HDN was positively correlated with the titer of maternal antibody IgG (r=0.96, P<0.01). There was no significant correlation between the time of jaundice and the anti IgG titer of ABO-HDN in children (P>0.05). The indications of phototherapy, the incidence of anemia in neonates within 1 week and exchange transfusion rate of mother anti-titers ≤1∶64 group were all lower than those in the other two groups (P<0.05). The incidence of anemia in neonates within 1 week and exchange transfusion rate of mother anti-titers ≥1 ∶256 group were significantly higher than those in the mother anti-titers =1∶128 group (P<0.05). Conclusion: For the blood type A or B blood newborns of type O blood mothers, the early detection of three items of hemolysis and antibody titer can predict the possibility and severity of hemolytic disease in newborns.
