中药治疗儿童肾病综合征的组方规律及网络药理学研究

摘要:
目的:挖掘中药治疗儿童肾病综合征的组方规律并进行网络药理学验证,为治疗肾病综合征提供用药参考。方法:检索 中国知网、万方数据库、维普数据库、中国生物医学文献数据库2012-2022 年收录的内服中药治疗儿童肾病综合征的相关文献, 使用Excel 2019、IBM SPSS Modeler 18. 0、SPSS 26. 0 软件对纳入中药的使用频数进行统计,并进行关联规则分析及聚类分析。 通过中药系统药理学数据库与分析平台( TCMSP) 数据库获取药物的活性成分,并收集其活性成分所对应的靶点,使用 GeneCards、OMIM、DrugBank 数据库获取儿童肾病综合征的靶点基因,通过STRING 平台构建蛋白相互作用( PPI) 网络,应用 MetaScape 平台分析“药物-成分-靶点”及其参与的生物学过程及作用通路,然后使用CytoScape 3. 7. 2 软件构建“ 药物-靶点-通 路”网络,最后进行分子对接验证。结果:纳入文献147 篇,得到内服中药方剂181 个,中药229 味,其中黄芪、茯苓等使用频数 较高。关联规则提示支持度>20%、置信度>80%的药对共10 对。网络药理学研究中得到治疗儿童肾病综合征的活性成分为槲 皮素、木犀草素、山奈酚、丹参酮ⅡA 等,核心靶点有PTGS2,其作用通路主要为炎症、免疫通路,分子对接显示结合活性较好。结 论:儿童肾病综合征发病机制复杂,中药用药以补气健脾、淡渗利湿、活血行气为主,可能通过抗炎、抗氧化应激、抗细胞衰老机制 治疗儿童肾病综合征。

Abstract:
Objective: To explore the formation law of traditional Chinese medicine in the treatment of children with nephrotic syndrome and perform network pharmacological verification, so as to provide reference for the treatment of nephrotic syndrome. Methods: CNKI, Wanfang, VIP and CBM databases were retrieved to collect literature related to traditional Chinese medicine in the treatment of children with nephrotic syndrome from 2012 to 2022. Excel 2019, IBM SPSS Modeler 18. 0, SPSS 26. 0 software were used to analyze the application frequency of traditional Chinese medicine, association rule analysis and cluster analysis were carried out. Active components of drugs were obtained through Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform ( TCMSP), corresponding targets of active components were collected. GeneCards, OMIM and DrugBank databases were used to obtain the target genes of nephrotic syndrome in children, and protein-protein interaction (PPI) network was constructed through STRING platform. MetaScape platform was used to analyze the “drug-component-target” and its involved biological processes and pathways, and CytoScape 3. 7. 2 software was used to construct the “drug-target-pathway” network, finally molecular docking was performed for verification. Results: A total of 147 articles were included, including 181 oral traditional Chinese medicine formation and 229 kinds of traditional Chinese medicine, among which Hedysarum Multijugum Maxim and Poria Cocos Wolf were commonly used. The association rules suggested that there were 10 drug pairs with support > 20% and confidence > 80%. The active components of traditional Chinese medicine in the treatment of children with nephrotic syndrome were quercetin, luteolin, kaempferol and tanshinone ⅡA. The core target was PTGS2, mainly for inflammatory and immune pathways. Molecular docking showed that the binding activity was good. Conclusion: The pathogenesis of nephrotic syndrome in children is complicated, traditional Chinese medicine is mainly used to tonify Qi and strengthen the spleen, lighten osmosis and dehumidification, and promote blood and Qi, which may be used to treat nephrotic syndrome in children through anti-inflammatory, anti-oxidative stress, and anti-cellular senescence mechanisms.
