| 目的：应用PDCA循环管理体系提高医院药房调剂质量，防范调剂差错。方法：我院2012年成立PDCA管理小组，每次运行PDCA循环管理体系历时4个月，2012年10月至2013年1月运行第一次，2014年1-4月运行第二次。从医院信息系统（HIS）中调取医院药房2012年5-10月、2013年2-7月、2014年5-10月三个时间段共2 454 986张处方，统计其调剂过程中出现的差错记录12 518起并进行分析。结果：经2次PDCA循环管理后，调剂差错率由0.85%（2012年5-10月）降低至0.10%（2014年5-10月）（P<0.01）。结论：PDCA循环管理体系能显著提高医院药房调剂质量，使调剂差错率降低到预定目标值，是防范调剂差错的有效工具。 |

| Objective: To study PDCA cycle management system was applied to improve dispensing quality of hospital pharmacy to reduce dispensing errors of hospital pharmacy. Methods: Tow times 24-month PDCA（plan, do, check and action）cycle managements were used to manage dispensing of hospital pharmacy and analysis to 12 518 dispensing errors. Results: After two times PDCA cycle managements, dispensing error rate drop from 0.85% to 0.10% and dispensing error rate ratio had significant improvement (P<0.01). Conclusion: PDCAcycle management system for pharmacy dispensing level has increased significantly, thus reduce dispensing error rate to the intended target, is an effective tool to avoid dispensing error. |
