不同剂量猪肺磷脂注射液治疗重症胎粪吸入综合征的疗效及安全性观察

摘要:
目的:探讨不同剂量猪肺磷脂注射液(固尔苏)治疗重症胎粪吸入综合征的疗效及安全性。方法:选取我院100例重症胎粪吸入综合征患儿,随机分为大剂量组和小剂量组,大剂量组给予猪肺磷脂注射液200 mg/kg,小剂量组给予猪肺磷脂注射液100 mg/kg,观察治疗前及治疗后2 h、24 h两组患儿血气指标变化情况,并比较两组患儿机械通气时间、住院时间、住院总费用及固尔苏使用情况。结果:治疗后2 h、24 h两组患儿pH、PaO2均升高、PaCO2均降低,且大剂量组血气指标改善较显著(P<0.05);大剂量组患儿机械通气时间、住院时间均低于小剂量组(P<0.05);住院总费用大剂量组小于小剂量组,但差异无统计学意义(P>0.05);大剂量组患儿固尔苏的使用次数少于小剂量组(P<0.05),使用总量也小于小剂量组,差异无统计学意义(P>0.05)。随访至2015年7月30日,大剂量组患儿的病死率与气漏发生率明显少于对照组(P<0.05)。结论:首次大剂量使用猪肺磷脂注射液治疗新生儿重症胎粪吸入综合征的临床疗效明显优于小剂量,可有效改善患儿的肺氧合功能,降低机械通气时间和住院时间,减少固尔苏的使用次数和总量,从而减少辅助通气的应用,值得临床的推广。

Abstract:
Objective: To investigate the effects of different dose of Curosurf for treatment of severe meconium aspiration syndrome efficacy and safety. Methods: 100 cases in our hospital for treatment of severe meconium aspiration syndrome in children, according to the given Curosurf dose, and the average divided into two groups, high dose group with Curosurf capacity of 200 mg/kg, low dose group was 100 mg/kg. The changes of blood gas indexes in two groups of patients before and after treatment, 24 h and 2 h were observed before and after treatment, and comparison of two groups of children with mechanical ventilation time, hospitalization time, hospitalization expenses and the use of curosurf. Results: After treatment of 2 h, 24 h, pH, PaO2 and PaCO2 were both increased in two groups, and was lower, and the blood gas indexes were significantly improved (P<0.05). The mechanical ventilation time and length of hospital stay in the large dose group were lower than those in the small dose group (P<0.05). Followed-up date, the incidence of mortality and gas leak in the large dose group were lower than those in the small dose group (P<0.05). Conclusion: This is the first large dose using Curosurf for treatment of neonatal meconium inhalation comprehensive syndrome clinical curative effect is obviously superior to low dose can effectively improve children’s pulmonary oxygenation function. To reduce the time of mechanical ventilation and hospitalization time, reduce the frequency of use and the total amount of curosurf, so it can reduce the application of assisted ventilation, it is worthy of clinical promotion.
