| 的:系统评价布洛芬与对乙酰氨基酚治疗儿童感染性发热的疗效和相关不良反应。方法:计算机检索PubMed、Embase、Cochrane Library、CNKI、VIP、万方和CBM等电子数据库,查找布洛芬与对乙酰氨基酚比较治疗儿童感染性发热随机对照试验(RCT),检索时限均从建库至2015年2月。对符合纳入标准的RCT,由两位评价员按Cochrane系统评价的方法,独立进行资料提取、质量评价并交叉核对后,应用RevMan 5.35软件进行Meta分析。结果:共纳入12个RCT,2 167例感染性发热患儿。试验组给予布洛芬治疗,对照组给予对乙酰氨基酚治疗。Meta分析结果显示,布洛芬与对乙酰氨基酚治疗儿童感染性发热的效果比较,体温在各个时间点降低的均数差值MD分别为1 h [MD=0.22,95% CI(0.01,0.43),P<0.05],2 h [MD=0.28,95% CI(0.11,0.45),P<0.01],4 h [MD=0.55,95% CI(0.23,0.88),P<0.01],6 h [MD=0.48,95% CI(0.29,0.68),P<0.01],所有时间点的体温降低值布洛芬均优于对乙酰氨基酚,差异均有统计学意义。试验组和对照组比较不良反应,差异无统计学意义[RR=1.08,95% CI(0.89,1.32),P>0.05],提示布洛芬和对乙酰氨基酚的常见不良反应发生率没有明显差异。结论:现有研究证据显示,布洛芬在治疗儿童感染性发热疗效方面要优于对乙酰氨基酚,不良反应两者之间无明显差异。 |

| Efficacy of Ibuprofen vs Acetaminophen for Treating Children's Fever Caused by Infection: a Meta-Analysis |

| Objective: To systematically review the effectiveness of ibuprofen vs acetaminophen in the treatment of children’s fever caused by infection. Methods: Randomized controlled trials (RCTs) about ibuprofen vs acetaminophen for treating children's fever caused by infection retrieved in databases of PubMed, Embase, Cochrane Library, CNKI, VIP, Wanfang data and CBM. Data from inception to Feb, 2015. Two reviewers independently screened literature, extracted data, and assessed methodological quality. Then meta-analysis was conducted using RevMan 5.3.5 software. Results: A total of 12 RCTs involving 2,167 cases were included. The trial group were given ibuprofen while the control were given acetaminophen. The results of meta-analysis showed, the overall estimate were at 1 h [MD=0.22, 95% CI (0.01, 0.43), P<0.05], 2 h [MD=0.28, 95% CI (0.11, 0.45), P<0.01], 4 h [MD=0.55, 95% CI (0.23, 0.88), P<0.01], 6 h [MD=0.48, 95% CI (0.29, 0.68), P<0.01], indicating significantly better fever control for ibuprofen at 1 h, 2 h, 4 h, 6 h postdose compared with acetaminophen. The adverse events did not find a statistically significant difference between ibuprofen and acetaminophen. Conclusion: Current evidence shows that Ibuprofen is more efficacious than acetaminophen for the treatment of fever caused by infection in children and is equally safe. |
