临床药师对我院儿科病区丙氨酰-谷氨酰胺注射液使用的干预效果

摘要:
目的:评价临床药师对我院丙氨酰-谷氨酰胺注射液使用情况进行合理性干预后的效果,分析干预后儿科病区使用丙氨酰-谷氨酰胺注射液仍存在的问题并对其进行二次合理性评价。方法:调取临床药师干预前后我院儿科病区使用丙氨酰-谷氨酰胺注射液的全部例数和各病区例数明细进行比较。采用分层随机抽样模式,抽取临床药师干预前后儿科病区使用丙氨酰-谷氨酰胺注射液病历各300 份,统计并比较溶媒选择和溶媒溶质体积比。将我院2017 年临床药师干预后,使用数量排名前5 的儿科病区,按照简单随机抽样模式,各抽取100 份病历进行回顾性分析并进行二次合理性评价。结果:2017 年丙氨酰-谷氨酰胺注射液的全年用量在临床药师对其进行合理干预后,同比下降18.39%。抽查的300 份病历中溶媒选择含氨基酸溶媒的比例由25.67%上升至73.67%,溶媒与溶质体积比逸5: 1 的比例由0% 上升至69.33%。结论:临床药师对丙氨酰-谷氨酰胺注射液的干预有效,但该药应用于儿童尚无权威参考依据,尤其在临床适应证和儿童用药剂量等方面急需建立适用于儿童的权威性专家共识。

Abstract:
Objective: To compare the effects of clinical pharmacists on the use of alanyl-glutamine injection in our hospital before and after rationality intervention, analyze the problems that still existing in the use of alanyl-glutamine injection in the pediatric ward after intervention, and conduct the second rationality evaluation. Methods: The number of all cases of alanyl-glutamine injection in the pediatric ward of our hospital before and after intervention by the clinical pharmacist was compared with the number of cases in each ward. A stratified random sampling model was used to extract 300 cases of alanyl-glutamine injection in the pediatric ward before and after intervention by clinical pharmacists. The media selection and solvent solute volume ratio were collected and compared. After intervention by clinical pharmacists in 2017, the top five pediatric wards ranked by consumption amount were collected, and 100 cases were selected from each ward for retrospective analysis and secondary rationality evaluation according to the simple random sampling model. Results: In 2017, the annual consumption amount of alanyl-glutamine injection decreased by 18.39% year-on-year after intervention by clinical pharmacists. Among the 300 cases selected for random examination, the proportion of solvent-containing amino acid solvent increased from 25.67% to 73.67%, and the ratio of solvent to solute volume ratio ≥5 :1 increased from 0% to 69.33%. Conclusion: The rational intervention by clinical pharmacists on alanyl-glutamine injection is effective, but there is no authoritative basis or reference for the use of this drug in children. In particular, it is urgent to establish the authoritative expert consensus for children in terms of clinical indications and child-related doses.
