| [摘要]目的:调查PCR确诊百日咳住院患儿及其密切接触者百日咳鲍特菌培养分离情况,并检测患儿分离株对9种抗生素的MIC值,为临床合理用药提供参考。方法:对2015年8月至2017年6月深圳市儿童医院PCR确诊的百日咳住院患儿及其密切接触者,采集鼻咽拭子进行百日咳鲍特菌培养,并对患儿分离株使用E-Test法检测红霉素等9种抗生素的MIC值。结果:PCR确诊百日咳住院患儿共373例,其中男223例(59.79%),年龄≤3个月200例(53.62%),经培养分离出百日咳鲍特菌136株(36.46%)。在449例密切接触者中,分离出116株百日咳鲍特菌(25.84%),主要来自母亲(50.00%,58/116)。E-Test法提示患儿百日咳鲍特菌对红霉素等大环内酯类抗生素的敏感率均为47.7%,对复方磺胺甲噁唑敏感率为100%。结论:百日咳住院患儿以3个月以下小婴儿多见;母亲可能是主要传染源;深圳地区百日咳鲍特菌对大环内酯类抗生素耐药较常见,未发现对复方磺胺甲噁唑耐药的菌株。 |

| [Abstract] Objective: To investigate the culture and isolation of Bordetella pertussis from hospitalized children diagnosed by Polymerase Chain Reaction (PCR) and the close contacts, and to detect the minimal inhibitory concentration (MIC) of 9 antibiotics, so as to provide reference for rational drug use in clinic. Methods: Nasopharynx swabs specimens were collected for Bordetella pertussis culture from hospitalized children with pertussis diagnosed by PCR and the close contacts in Shenzhen Children’s Hospital from Aug. 2015 to Jun. 2017. MIC of 9 antibiotics such as erythromycin were detected by E-Test for Bordetella pertussis isolates from children. Results: Totally 373 hospitalized children were diagnosed as pertussis, including 223 males (59.79%), 200 cases (53.62%) aged ≤3 months, 136 strains of Bordetella pertussis (36.46%) were isolated after culture. Among the 449 close contacts, 116 strains of Bordetella pertussis (25.84%) were isolated, mainly from mothers (50.00%, 58/116). The sensitivities of Bordetella pertussis to macrolides and sulfamethoxazole-trimethoprim were 47.7% and 100%, respectively. Conclusion: Most hospitalizations with pertussis occurred in infants younger than 3 months. Mothers may be the main source of infants infected by Bordetella pertussis. The resistant Bordetella pertussis isolates to macrolides were common in Shenzhen and no resistance to sulfamethoxazole-trimethoprim isolate was found. |
