阿奇霉素静脉滴注联合布地奈德雾化吸入治疗儿童闭塞性细支气管炎临床观察

摘要:
目的:探讨阿奇霉素静脉滴注联合布地奈德雾化吸入治疗儿童闭塞性细支气管炎对于改善患儿临床症状的效果。方法:选取2015年1月至2016年1月在我院进行治疗的闭塞性细支气管炎患儿70例,随机分为观察组和对照组各35例。观察组患儿采用阿奇霉素静脉滴注联合布地奈德雾化吸入进行治疗,对照组患儿采用布地奈德雾化吸入进行治疗,比较两组患儿的治疗总有效率、气喘发生率和感染发生率,治疗前后的喘息得分、临床表现总得分,治疗前后血清白细胞介素4(IL-4)、γ干扰素(IFN-γ)水平。结果:观察组总有效率为91郾43%,显著高于对照组的71郾43%,差异有统计学意义( P<0.05)。治疗前,两组患儿的喘息得分、临床表现总分比较差异无统计学意义(P>0.05),治疗后观察组患儿的喘息控制得分为(1.52±0.41)分、临床表现得分为(17.68±2.41)分,低于对照组的(2.21±0.75)分和(21.03±2.75)分,差异有统计学意义(P<0.05)。观察组患儿治疗期间气喘发生率(11.43%)和感染发生率(8.57%)低于对照组患儿(P<0.05)。治疗前两组患儿的IL-4、IFN-γ水平比较差异无统计学意义(P>0.05),治疗后观察组患儿IL-4水平显著高于对照组[(52.1±2.5)pg/mL vs (41.3±4.6)pg/ mL,P<0.05],观察组患儿的IFN-γ水平显著低于对照组[(134.5±14.2)pg/mL vs (152.5±12.4) pg/mL,P<0.05]。结论:阿奇霉素静脉滴注联合布地奈德雾化吸入治疗儿童闭塞性细支气管炎的疗效显著,能够很好的改善患儿的临床症状,降低患儿的炎症指标水平,值得临床推广应用。

Abstract:
Objective: To evaluate the efficacy of azithromycin combined with budesonide aerosol inhalation in children with occlusive bronchiolitis. Methods: Seventy children with occlusive bronchiolitis were chosen from January 2015 to January 2016 in our hospital, all children were randomly divided into observation group (n = 35) and the control group (n=35). The observation group was given azithromycin combined with budesonide aerosol inhalation, the control group was given budesonide aerosol inhalation only. The total effective rate, the incidence of asthma and infection before and after treatment and the wheezing score, clinical manifestation score before and after treatment, the serum interleukin 4 (IL-4) and interferon gamma (IFN-γ) levels were compared in two groups. Results: The total effective rate of the observation group was significantly higher than that of the control group (91.43% vs 71.43%, P<0.05). There were no significant differences in the scores of wheezing and clinical scores between the two groups before treatment, but after treatment there were significant differences in two groups (1.52±0.41 vs 2.21±0.75, 17.68±2.41 vs 21.03±2.75, P<0.05).There were no significant differences in IL-4 and IFN-γ levels between the two groups before treatment (P>0.05). During therapy, the incidence of asthma and infection in observation group were significantly lower than those in control group (P<0.05). After treatment, the IL-4 level in the observation group was significantly higher than that in the control group ((52.1±2.5) pg/mL vs (41.3±4.6) pg/mL, P<0.05), the level of IFN-γ in the observation group was significantly lower than that in the control group ((134.5±14.2) pg/mL vs (152.5±12.4) pg/mL, P<0.05). Conclusion: Azithromycin combined with budesonide aerosol inhalation in the treatment of occlusive bronchiolitis in children can improve the clinical symptoms, it is worthy of popularization and application.
